  • My child is struggling to learn and retain new information

  • My child is struggling to keep pace with their peers

  • My child seems to think, behave, or react in ways that are different from most of their peers

  • My child’s teachers have expressed concerns about their academic or social performance

  • My child is having a hard time making or maintaining friendships or is otherwise struggling in social interactions

  • My child seems especially nervous, anxious, or worried, and it’s impacting their daily life functioning

  • My child has a history of neurological insult/injury

  • My child has a history of developmental delays

  • My child’s medical history has impacted their cognition in some way and I’d like to better understand how

  • My child is struggling, and I’m not sure why

  • My child’s pediatrician, therapist, or other outpatient provider recommended that we seek an evaluation

  • My child was recently evaluated by their school, but I’d like to get a more comprehensive picture of their strengths and weaknesses

  • My child was previously evaluated, and I’d like to get an update on how they're doing and any changes in their thinking skills, neurocognitive abilities, or achievement
